The doctors you keep
Names of the providers you would not want to give up. Networks are the first thing I check, and knowing who matters saves a round of questions.
The medicine you take
A rough list of prescriptions, including anything seasonal. Drug lists differ enough between plans that this alone can settle which one wins.
What changed lately
